Medication Policy
Student are NOT allowed to carry any medications at school ( including over the counter medicine) unless there is a doctor's order. When it is essential for your child to receive medication at school, it may be given if the following procedures are met.
1. Have doctor completed one of the medication forms. See attached.
2. Bring the signed medication form and the medication bottle from pharmacy to the Health Office.
The medication bottle should have all the following information:
Child's name
Name of medication
Doctor's name
Doctor's phone number
Expiration date of medication
3. A new medication form is required each school year. Please renew them yearly at the beginning of the school year.
