^Important:

^{1. Every new school year, an athlete is expected to complete a new Athletic Clearance. This includes returners (10th-12th graders) to MKHS.}

^{2. Physicals are good for one year on the date issued. Reuse paperwork for the new Athletic Clearance if it has not expired.}

^{3. Make sure that physicals are always up to date. If a physical expires during the season or at any time, the athlete will be forced to step away from the team until the physical is updated and cleared by the nurse. If a new physical is uploaded during the school year, please go speak or email the nurse to get re-clearance.}
